Support going home from hospital

'Just Got Home' service

We understand that some patients may have concerns about returning home after a hospital stay.

Available to inpatients at Lymington New Forest Hospital, Just Got Home is our free service which aims to support you and help you feel comfortable, safe and independent in your home.

We meet patients and their families on the ward and follow up with a home visit. Together, we identify what support might be necessary at home to maintain or increase your independence, safety and quality of life. We then help to put those things in place.

This is a FREE service supported with funding from New Forest District Council.

Your home visit

Patients wishing to use this service will be contacted shortly after discharge from hospital to arrange a convenient time to visit. Home visits are usually within 2 weeks after arriving home.

In most cases, patients will be visited by the person they met whilst on the ward, so they will know exactly who to expect.

You do not need to have a disability to use the service.

The home visit will provide information and can help to arrange access to services on your behalf. During the visit, we can provide information on:

• Transport solutions
• Equipment
• Alarms
• Welfare benefits
• Support groups
• Befriending
• Lunch clubs
• Fire and home safety checks
• Trade services
• Home care/domestic support agencies
• Priority services register
• Home adaptations

Independence Matters

Through our Independence Matters service, we can provide advice on home adaptations and eligibility for Disabled Facilities Grants, and kick-start the application process for you.
